Transgender federal inmates are facing uncertainty and fear following an executive order by President Donald Trump mandating prison placements based on biological sex rather than gender identity.
The directive, signed on Trump’s first day in office, reverses previous policies and prohibits federal funding for transition-related healthcare.

It has raised serious concerns about the safety of transgender inmates, especially transgender women being transferred to male facilities.
“It’s going to be incredibly dangerous,” said attorney Moira Meltzer-Cohen, who represents Donna Langan, a transgender woman serving a life sentence for armed robbery.
Meltzer-Cohen warned of potential violence, calling the situation a “bloodbath” waiting to happen.
The Bureau of Prisons, which oversees over 2,000 transgender inmates, has yet to determine how placements will be handled for individuals who have undergone gender-affirming surgeries.

The agency cites additional security concerns as part of its decision-making process.
Legal advocates and defense attorneys are already challenging the policy. Federal public defender Sandra Gant revealed that one of her transgender clients was segregated in preparation for transfer to a male prison. “She is terrified,” Gant said, urging immediate legal strategies to protect trans inmates.
While the policy has drawn criticism, some inmates, like Rhonda Fleming in Texas, have expressed support.
Fleming, who unsuccessfully sued to exclude trans women from women’s prisons, claimed some male inmates exploit gender identity to gain access to female facilities.
The executive order has drawn widespread backlash as part of Trump’s broader rollback of transgender protections.
